Book excerpt: Multiple voices throughout the last century have preached the merits of various treatments for schizophrenia, ranging from cold baths to the currently accepted standards such as neuroleptic medication. Along with these ongoing treatments, there have been quiet commentaries, made mostly from the sidelines, suggesting the need to shift and refocus the way we think and talk about schizophrenia. Harry Stack Sullivan noted in 1927 that, 'The psychiatrist sees too many end states and deals professionally with too few of the pre psychotic" (Sullivan 192711994, p. 135). Similar thoughts have been echoed by purveyors of modem treatment for psychosis such as Thomas H. McGlashan: "Like others before me, I tried to make a difference . . . but like the others my efforts were largely in vain. I came upon the scene too late; most of the damage was already done" (McGlashan, 1996). Similar interest in the early phase of schizophrenia has developed across the globe and consolidated into a tentative, yet meaningful deliberation about the potential for prevention of psychotic illness through early identification and intervention. In the past decade, international support has grown from: Ian Falloon's prodromal intervention project in Great Britain (Falloon et aI. , 1996); Patrick McGorry's and Jane Edward's first episode psychosis program in Melbourne, Australia (McGorry et al.

Book excerpt: Evidence indicates a correlation between early intervention and improved response in psychiatric disorders. Yet the majority of evidence is from high-income countries, and there is a lack of evidence and resources on the impact of early intervention across cultures and in low- and middle-income countries. Early Intervention in Psychiatric Disorders across Cultures aims to bridge this gap by focusing on the evidence and methods of intervention in less affluent countries. It covers mood disorders, personality disorders, addictions, and other psychiatric conditions, whilst most research has focused on psychoses and bipolar disorder. Over 17 chapters, this resource guides the reader through an array of approaches to early intervention, from brain imaging and available care pathways, to cultural and societal factors. Part of the Oxford Cultural Psychiatry series, this resource offers readers a picture of the current health care pathways available as well as social, cultural, and ethical considerations to apply in practice.

Book excerpt: Psychotic Disorders: Comprehensive Conceptualization and Treatments emphasizes a dimensional approach to psychosis--one of the most fascinating manifestations of altered brain behavior--that cuts across a broad array of psychiatric diagnoses from schizophrenia to affective psychosis and organic disorders like epilepsy and dementias. Written by an international roster of over seventy leading experts in the field, this volume comprehensively reviews, critiques, and integrates available knowledge on the etiology, mechanisms, and treatments of psychotic disorders, and outlines ways forward in both research and clinical practice towards more objective, mechanistically-based definitions of psychotic disorders. Chapters address topics such as psychosis phenomenology, biomarkers and treatments, the overlaps and interfaces between psychiatric disorders within the psychosis dimension, and novel disease definitions. Furthermore, the volume incorporates findings on potential mechanisms, bridges between various system levels (i.e., genetic, epigenetic, molecular and cellular, brain circuit and function, psychological, social, environmental and cultural) and their interactions, as well as the potential role in causation and/or mediation in psychotic disorders. Finally, the volume outlines a broad array of treatment approaches, from the readily available (e.g., psychopharmacology, various modalities of psychotherapy) to the experimental (e.g., cognitive interventions, neuromodulation). With a concluding section of forward perspectives conjecturing future directions and related challenges, this book aspires to stimulate new knowledge, generate novel frameworks, and carry new directions forward on psychotic disorders.

Available in PDF, EPUB and Kindle. Book excerpt: Traditionally, people with psychotic symptoms have been treated with anti-psychotic or neuroleptic drugs. While this approach is beneficial to a number of people, there are many for whom it is problematic. Recent recognition of these problems has led to the development of effective complementary treatments of a specifically psychological nature. In Cognitive Behavioural Interventions with Psychotic Disorders leading researchers and practitioners in this area provide a comprehensive overview for all those undergoing related training in psychology and psychiatry, as well as nursing and social work. The book provides a general background to cognitive treatment, and also discusses specific uses of the therapy in treating those who have hallucinations, as well as those with delusions and schizophrenia. The contributors also suggest how cognitive behavioural approaches can be integrated with other strategies such as pharmacological methods, or in the context of the family.

Book excerpt: This book provides clear and concise guidance for clinicians when they encounter a patient with psychosis, starting with the medical work-up to arrive at a diagnosis and ending with the comprehensive care for patients with established schizophrenia. It covers the optimal use of medications (emphasizing safe use) but also addresses other treatment approaches (psychological treatments, rehabilitation) and the larger societal context of care, including how to work effectively in complex systems. It uniquely condenses the literature into teaching points without simplifying too much, effectively serving as a learning tool for trainees and professionals. For this second edition, the book was extensively updated and its content expanded, with new figures as well. Each chapter begins with an initial summary and includes Tips and Key Points in text boxes. Each chapter also includes links to external websites and additional readings. The book contains clinical and practical wisdom for clinicians who are treating real patients at the front lines, setting it apart from all other texts. Psychotic Disorders is an excellent resource for medical students, early career professionals such as trainees and fellows, and related clinicians seeking additional training and resources, including those in psychiatry, psychology, neurology, and all others.

It pays particular attention to early intervention programmes established in Hong Kong and Singapore, and assesses recent developments in Korea, Japan and other countries. The volume covers approaches in the management of psychosis, including pathway to care, stigma and interventions. With reference to the experiences of frontline practitioners, research findings and theories, it highlights the practical needs in non-Western healthcare settings. Culturally relevant discussions on recovery, relapse, self-harm and comorbid substance abuse are discussed. It also covers case studies to illustrate challenges and strategies in managing early psychosis.

Book excerpt: Despite recent advances in pharmacotherapy, psychosocial casework, and early interventions, improving the treatment of psychotic disorders is still a major challenge for mental health professionals. The coaching approach discussed in this book specifically addresses that difficulty. It is a recovery-oriented approach that attempts to help mental health patients function better in everyday life. The book starts with a short introduction to psychotic disorders, the principles of psychosocial intervention, and the challenges inherent in such interventions. It then provides a systematic review of the lifestyle redesigning approach, i.e., the coaching approach in the context of an early psychosis intervention program in Hong Kong (the Jockey Club Early Psychosis Program). There is also a step-by-step guide to conducting group intervention sessions using the coaching approach. Intended for mental health professionals working with psychosis patients, Coaching Intervention for Psychosis: A Lifestyle Redesigning Approach provides useful instructions for basic training. It is also invaluable to patients and caregivers who wish to understand and facilitate the intervention procedures.

Book excerpt: "The early recognition and treatment of psychiatric disorders constitutes secondary prevention: to date, the best evidence and best practice has been in EI for psychosis in young people. This new title will inform interventions in people from all age groups, building on the core components of excellent services: holistic, more engaging, low impact, culture- and age-sensitive practices, robust crisis interventions, assertive case management, flexible home visiting, family consulting, and in- and out-of-hours active response services. EI Teams have a low threshold to identify cases, reduce stigma in patients and their local community, engage users and carers in services even if treatment is not indicated or not available, and vigorously treat the first signs of the disorder in the first three years ("the critical period"). In managing a complex mix of possible noncases and cases, medication is only one option and part of phase-specific psychosocial interventions: education, family work, cognitive behavioural therapy, assertive community treatment, substance misuse and vocational interventions, amongst others"--Provided by publisher.
